NOTICE TO APPLICANTS

Applicants applying for Firearms Purchaser Identification Cards, Replacement ID Cards, Change of Address or Name on ID Cards, or Permits to Purchase Handguns should expect a delay in the processing of their applications at the current time. These delays are residual effects from the COVID19 closure of IdentoGO Fingerprint Centers (Considered Non-essential businesses) during the Spring months which severely delayed the processing of applications submitted during those months. Also contributing to the delay is a 500% increase in applications (both Statewide and in Hazlet) since June 1, 2020. 

Applicants can help us in the processing of their applications by reading the following information to avoid making errors or omitting a required step that the applicant must take to complete the application process. Errors and omissions by applicants will require extra time to address by our investigators and add additional waiting time before your application is approved. Some errors cannot be corrected by our investigators and will require the applicant having to file a new application, which will add time onto your wait.

Due to the current backlog, it may be weeks before investigators will be able to examine your application.  They will contact you and advise you of incorrect or missing information at that time, with the possibility of you having to file a new application to correct the errors.

Applicants are advised that this application is an official government document. Providing incomplete and incorrect information will cause your application to be delayed. Providing false information will result in denial of your application and possible criminal penalties (N.J.S. 2C:39-10c).

The Police Department has added additional personnel to help with the backlog in applications. We are making progress in reducing the waiting time. We will update this site on a regular basis to keep you advised of any future developments.

Due to several hundred applications being submitted in the past several months and the additional workload placed on our Firearms Unit, we are unable to return phone calls to every applicant inquiring about the status of their application. Emails will be responded to without delay.

 

APPLICATION PROCEDURES FOR OBTAINING FIREARMS IDENTIFICATION CARDS AND PERMITS TO PURCHASE HANDGUNS

The following information is intended to explain the process for obtaining Firearms Purchaser Identification Cards and Permits to Purchase Handguns from the Hazlet Township Police Department. Many residents visiting this site are first time gun buyers, while others are experienced firearms owners who understand the purpose and procedure for obtaining the various firearms documents. Whether you are obtaining a firearm for the first time, a new resident to New Jersey (and Hazlet Township), or a resident that has previously applied for either Firearms Purchaser Identification Cards or Permits to Purchase Handguns through our agency, we would request that all visitors to this site read the information provided in its entirety before proceeding with the application process. There have been many recent changes in the application process on both the local and state levels which you must be aware of to allow us to process your application with as minimal a delay as possible.

This page has been updated to include the recent changes to procedures for Firearms Applications that were instituted on October 1, 2020.

TRANSITION TO NEW JERSEY STATE POLICE ON-LINE FIREARMS APPLICATION AND REGISTRATION SYSTEM (Also known as FARS).

In late 2019 the Hazlet Township Police Department began using the New Jersey State Police FARS on-line application system. This replaces the previous procedure where residents dropped off completed paper applications and other required documents at Police Headquarters. The previous paper STS-033 application form has been replaced by an on-line STS-033 form for applicants to complete. The only time you will be required to appear in person at Police HQ will be to pick up your completed Firearms Purchaser ID Cards and Permits to Purchase Handguns. All Police Departments in the State of New Jersey have been asked to transition to the FARS System to make it easier for applicants to apply for firearms permits and ID cards. A link to the NJ State Police FARS Site is provided further down this page.

The Firearms Application and Registration System (FARS) is used for applying for the following firearms related documents:

  • Permits to Purchase Handguns
  • Firearms Purchaser Identification Cards (Initial Issue)
  • Replacement of Lost or Stolen Firearms Purchaser Identification Card
  • Replacement of Mutilated Firearms Purchaser Identification Card
  • Change of Address on Firearms Purchaser Identification Card
  • Change of Name on Firearms Purchaser Identification Card
  • Change of Sex on Firearms Purchaser Identification Card

Applications for Permits to Carry Firearms are not available on the FARS Site.

 

TYPES OF DOCUMENTS THAT CAN BE REQUESTED AND THEIR PURPOSE

Many first-time applicants are unaware that there are 2 separate documents required to purchase various weapons and ammunition in New Jersey. Below is an explanation of purpose of each document, expiration dates, and cost.

Firearms Purchaser Identification Card

              A Firearms Purchaser Identification Card is required in the State of New Jersey for the purchase of the following:

    • Shotguns and Rifles
    • Ammunition (For Rifles, Shotguns, and Handguns)
    • Currently there is no expiration date for a Firearms Purchaser ID Card.
    • Current cost for a Firearms Purchaser Identification Card is $5.00.

Permit to Purchase Handgun

  • New Jersey Law requires a separate permit to purchase each handgun. This permit is solely for the purchase of a single handgun. It does not permit the applicant to purchase ammunition for that handgun.
  • A Permit to Purchase a Handgun expires 90 days after date issued by the Police Department. It can be renewed for an additional 90 days. At the current time permits are issued with both the initial 90 days included along with an additional 90 days, so there is no need for an applicant to come to the Police Department to have the permit renewed. Once 180 days have expired since the permit was issued, it is no longer valid and cannot be renewed.
  • Applicants that applied for a Permit to Purchase a Handgun prior to October 1, 2020 will receive a completed paper permit from the police department when the permit is approved.
  • Applicants that apply for a Permit to Purchase a Handgun after October 1, 2020 will receive an Electronic Permit (known as an E-PERMIT) instead of the current paper permit. This E-PERMIT will be issued upon approval of your application by the Chief of Police. An E-PERMIT is a digital link to the permit on the FARS website. The applicant will provide this link to a Firearms Retailer or other person from whom a weapon is being purchased. The Firearms Retailer/Seller will then activate the permit on the FARS website, enter the required information on the weapon purchased, and complete the purchase. All permit records will be maintained on the FARS Website. E-PERMITS will be valid for 180 days from date of issue. Once they expire after 180 days they cannot be renewed.
  • Current cost of a Permit to Purchase Handgun is $2.00.
  • First Time Applicants for a Permit to Purchase a Handgun should also apply for a Firearms Purchaser Identification Card on the same application if they do not already possess one. This is recommended by the NJ State Police when applying on the FARS Website as the best practice. It will save the applicant additional processing fees that will be incurred if they file two separate applications.

The following functions that were previously completed at the Hazlet Township Police Department are now done on the FARS website for all applicants (both first time applicants and returning applicants):

TO CORRECTLY COMPLETE THE ON-LINE FIREARMS APPLICATION, YOU WILL NEED TO HAVE THE FOLLOWING INFORMATION AVAILABLE:

  • ORI Number of the Hazlet Township Police Department which is NJ0133900. (You will not be able to complete the application without using this ORI Number. An incorrect ORI Number will send your application to the wrong police department and result in the applicant having to re-apply with the correct ORI).
  • Personal information including all former names and name aliases. (You should have your NJ Driver’s License available as the name on the application should match your Driver’s License EXACTLY)
  • Your SBI Number if you have previously been fingerprinted for firearms.

(The SBI number is located on the front of your Firearms Purchaser Identification Card)

  • Two references with their contact information

(Names, Addresses, Phone number, and correct email address. Your references will receive a reference form link sent to their email address. References should be reputable adults that are not related to the applicant.

  • Past residential history during the past 10 years. (Failure to list 10 years of prior addresses will delay your application)
  • For Change of Name Application ONLY- copy of supporting marriage license or court order (in any of these formats-PDF, JPG, JPEG, PNG, TIFF, and TIF).
  • Alien Registration Number if not a US Citizen.
  • Employer Contact information if employed with contact number and name of person to contact for employment verification.

 

Hazlet Township Police Firearms Investigators cannot change any information on your application with one exception: If you have entered incorrect emails or other information for your references, the police department investigators can correct that information on the FARS site and can then resend the emails to your references. Corrections to email addresses or contact information for references can be sent to Firearms@HazletPD.org.  Your application confirmation number must be included in the email requesting the corrections.

GUIDE FOR COMPLETING FIREARMS APPLICATIONS

The STS-033 Online Application is divided into 8 sections. These sections are:

Applicant MUST utilize the correct ORI (NJ0133900) for the Hazlet Township Police Department. You will not be able to complete the application without using this ORI Number. An incorrect ORI Number will send your application to the wrong police department and result in the applicant having to re-apply with the correct ORI.

Hazlet Township Police are only authorized to issue firearms documents to residents of Hazlet Township. Residents of other municipalities must apply to their own police department or to the NJ State Police if they provide police coverage in your municipality.

FARS will provide the following choices. More than one choice can be selected.

  • Initial Firearms Purchaser Identification Card
  • Permit to Purchase A Handgun
  • Lost or Stolen Firearms Purchaser Identification Card
  • Mutilated Firearms Purchaser Identification Card
  • Change of Address on Firearms Purchaser Identification Card
  • Change of Sex on Firearms Purchaser Identification Card
  • Change of Name on Firearms Purchaser Identification Card

First Time Applicants for a Permit to Purchase a Handgun should also apply for a Firearms Purchaser Identification Card on the same application if they do not already possess one. This will avoid additional waiting time and fees if you apply in a separate application.

For Change of Name Application ONLY- copy of supporting marriage license or court order (in any of these formats-PDF, JPG, JPEG, PNG, TIFF, and TIF). These can be uploaded to the FARS site before submitting the application.

NAME

Information in the application should match EXACTLY that information that is contained on the applicant’s NJ Driver’s License (Name, Address, Date of Birth). Firearm’s Retailers request a Driver’s License as Photo Identification when purchases are being made. If the information on your Driver’s License does not EXACTLY match your Firearms Identification Card, the Federal NICS (National Instant Criminal Background System) check that is required at time of purchase will be denied and you will not be able to complete your purchase.

Applicant should not use shortened versions or their names such as “Joe” instead of “Joseph” or “Sue” instead of “Susan”

 

DATE OF BIRTH ERRORS (VERY IMPORTANT) When applying on the FARS site and entering a Date of Birth, a drop-down calendar appears and defaults to a date 18 years in the past. (Example: If today is October 15, 2020, the default date that appears will be 10/15/2002). This is designed to prevent applicants under 18 from applying for Firearms ID Cards as they are legally unable to purchase weapons and are prohibited by NJ State Law from obtaining a Firearms Purchaser Identification Card. Any attempt to enter a date of birth for an applicant under age 18 will not be accepted.

A frequent problem with the drop down calendar is that applicants older than 18 often rush through the application form and fail to accurately enter their correct year of birth after entering their month and day of birth, causing the FARS System to default to a year 18 years ago. (In 2020 it will default to 2002).

Applicants are strongly advised to inspect your application for all correct information on the REVIEW step of the application process.

If you find that you have entered an incorrect Date of Birth on a submitted application, please contact our investigators as soon as possible by email at Firearms@HazletPD.org DO NOT File a replacement application as it will only lead to additional problems further in the investigation process. The application with the incorrect date of birth may need to be withdrawn from the FARS site before a replacement application can be submitted.

SOCIAL SECURITY NUMBER

Disclosure of Social Security Number and Driver’s License details are voluntary. However, without it, the processing of the application may be delayed. This information is considered confidential.

Applicants are required to list all residential addresses for the previous 10 years. Omitting any prior addresses will delay the processing of your application or may result in applicant having to submit a new application.

Our investigators are required to verify the applicant’s residential history during the investigative process.

Applicants are advised that our investigators use numerous databases to verify residency history. NJ DMV records reveal when a Driver’s License was initially issued, along with any address changes made in the past years.

Property Tax Records are also used to verify residences.

There are 12 questions that must be answered in accordance with NJ Firearms Law (N.J.S. 2C:58-3 Purchase of Firearms).

We are unable to process your application unless all 12 questions are answered.

Two References with names, addresses, phone numbers, and email addresses. To process the application the municipal police department requires two references from reputable persons who are presently acquainted with the applicant, other than relatives. Not having the feedback can delay the processing. The FARS service sends two reminder emails with the link to submit electronic feedback.

Please advise your references that the FARS system will email them the reference questions to be answered. Applicants will receive email updates from the FARS site when your references complete the reference form.

Applicants have filed duplicate applications because they made errors on the email addresses for their references on a previous application. If you make a mistake on a reference’s email, please email our Firearms Applicant Unit at firearms@HazletPD.org with the changes you want to be made with to information on your references. You must include the Confirmation Number from the top of your application in your email. Our personnel have access to the FARS site and can change email addresses or change names of references. Once they change the email or reference names, our personnel can resend the reference questions to the corrected email address.

One of the last steps on the FARS site before submitting your application is a REVIEW page, which prompts you to review all the information you previously entered and allows you to change any previously entered information.

Applicants should consider the REVIEW step as the MOST IMPORTANT part of the application process.

Errors that are found at this step and corrected will allow us to process your application as quickly as possible.

Errors that are not corrected at this step will delay your application and possibly require you to file a new application at some point in the future, which could delay your application process by months.

Make sure your name is entered EXACTLY as it appears on your Driver’s License. Address must also match your driver’s license EXACTLY

Make sure your Date of Birth is correctly entered.

 

Prior to going to the FARS site to start your application, it is strongly recommended that applicants read the GUIDE FOR COMPLETING YOUR APPLICATION and TIPS FOR COMPLETING APPLICATION/RESPONSIBILITIES AFTER SUBMITTING YOUR APPLICATION listed on this page to prevent common errors by applicants which have been found to delay the processing of firearms applications. Errors on applications cannot be corrected by either Hazlet Police Personnel or the NJ State Police Firearms Unit. Applicants with errors on their applications will be required to file a new application. Fees paid in the initial application will not be refunded.  Applicant will also be delaying their application process by several months as applications are investigated in the order that they are received on the FARS Website.

  • The final step prior to submitting your application is a DISCLAIMER/MENTAL HEALTH SEARCH AUTHORIZATION.
  • Applicant certifies that all answers are complete, true, and correct in every particular, and that if any of the answers are false, applicant is subject to punishment. Falsification of this form is a crime of the 3rd Degree as provided by N.J.S. 2C:39-10c.
  • Applicant consents to a search of New Jersey Mental Health Records, and the disclosure of those records if found, including disclosure of the fact that said records may have been expunged. Applicant agrees to disclosure of those records to the Chief of Police or the Superintendent of State Police or their designates, for the purpose of verifying the applicants firearms permit application and their fitness to own a firearm under N.J.S. 2C:58-3.

  • Applicants acknowledge that they will be required to pay $5.00 for the initial firearms purchaser Identification Card and $2.00 for each Permit to Purchase a Handgun.
  • The applicant should print both the confirmation page and application at the end of the process. This is for quick reference in the event of an issue. Make sure you record the date of your application and have your confirmation number available as it is the easiest way to search for your application on the FARS site should issues occur.

 

 

TIPS for Completing Application and Responsibilities After Submitting Your Application

 

  1. The Applicant is required to take further actions before the Hazlet Township Police Department can start the processing of your application. These would depend on the selected purpose of your application. The required actions for each applicant are listed in the “Next Steps” section of the Confirmation Email.
  2. Mental Health Records Search- VERY IMPORTANT- PLEASE READ Applicants that are new residents to New Jersey and/or residents that have not resided in New Jersey for the past 10 years continuously will be required to complete a paper copy of a Consent for Mental Health Search form (SP 066). Prior New York Residents are also required to fill out NY Office of Mental Health Form OMH11 for NY Searches. A link to the form is provided below.
    • The New Jersey Request for Mental Health Search form can be downloaded from the NJ State Police Firearms Website. This form is also available at the Records Department window located in the lobby of Hazlet Police HQ. One completed form will be required for each state in which the applicant resided during the past 10 years. Follow the instructions on the form to complete it properly.
    • This form must be turned into the Hazlet Township Police Department as close as possible to the date when you applied on the FARS site. Failure to turn this form at the time you file your application will result in delays in processing your application.
    • Do not sign this form until you are in the presence of Police Department personnel at HQ. The Hazlet Police Firearms Investigators will submit the form to the proper authorities in the previous states that you have resided.

  1. Applicants that have not been fingerprinted for firearms purposes in NEW Jersey before will need to make an appointment with IdentoGO to get fingerprinted. Information required for fingerprint appointment is included in the Confirmation email. Fingerprint appointments can be scheduled at:

  1. A fee is paid by applicant to the IdentoGo fingerprint vendor when setting up the appointment on-line. This fee covers both the fee for fingerprinting and the fee for the criminal history check.
  2. The nearest IdentoGo offices are located at 1817 Highway 35, Unit 5, Oakhurst, NJ, 07755, and at 2660 Woodbridge Ave, Edison, NJ 08837. Both offices are about 13 miles from Hazlet. There are other offices located throughout the state.
  3. When applying on the IdentoGo site to set up your fingerprint appointment, please make sure you enter your name exactly as it appears on your FARS Application. Errors may result in your fingerprint results not being matched up with your application.
  4. Delays in setting up an appointment for having your fingerprints scanned will delay the processing of your application. Investigation of new applicants cannot start until fingerprints have been obtained and results posted on the FARS site.
  5. No payment is taken from First Time applicants by FARS. Only 212A applicants (those applicants that have previously been fingerprinted and have a state issued SBI Number) will be required to pay at the end of their online application process. This is a service fee required by the State Police for conducting a Criminal History Check.
  6. Applicants are going to receive several Firearms Application and Registration System (FARS) generated emails at various stages of the application process. One email will advise you that your Criminal Background Check has been completed. You will receive this within a few days after getting your fingerprints taken. Previous applicants will receive this email when the NJ State Police complete a check of their records based on the SBI Number that was submitted with your application.
  7. These FARS generated emails DO NOT indicate the status of the investigative steps that must be conducted by our investigators. Our investigators cannot start their part of the investigative process until the NJ State Police complete these Background Checks. Many applicants mistakenly believe that these emails indicate the completion of the entire investigative process. Our department must conduct numerous other NJ State required checks of an applicant’s background on various NJ State Criminal Justice related sites. These sites include NJ Motor Vehicle Records, NJ Statewide Domestic Violence Records, NJ Statewide Juvenile Records, NJ Statewide Superior Court and Prosecutor Records, NJ Statewide Municipal Court Records, NJ Statewide Traffic Court Records, and other County and Local Police Computer Records. Verification of the past 10 years residential history is also required. These numerous checks take a large amount of time for each applicant, and must be conducted for every applicant, both first time applicants and previous applicants, even if the purpose of their application is to just change the address on their Firearms Purchaser Identification card.
  8. All Applicants – Contact your references to alert them to the emails that will be sent from the FARS site with a reference questionnaire. Both references must complete and return the questionnaire.
  9. Previous instructions on the FARS site may advise you that if you make an error on an application, you just need to file a new application. This is no longer recommended by the NJ State Police. Our Firearms Applicant Investigators have found numerous problems with applicants filing a second or third application to correct errors in their first application. The ideal situation is to completely check the information in your application before submitting it on the FARS site.
  10. If you have filed a duplicate application due to errors in a previous application, contact our firearms investigators at Firearms@HazletPD.org as soon as possible. We have seen numerous issues in the filing of duplicate applications that will delay processing of applications.
  11. You may complete the application using a smartphone, mobile device, laptop, or desktop computer.
  12. Upon Final Approval or your application by the Chief of Police, the FARS system will send you an email indicating your application has been approved. Please do not contact the Police Department when you receive this email. Upon approval of the application by the Chief of Police, it may take several days depending on volume to print out the Firearms ID Cards and Permits to Purchase Handguns as these are not automatically generated by the FARS system. Printing out the Firearms ID Cards and Handgun Permits is a complicated process. The information on each applicant must be manually entered into the templates for each document, as currently the FARS System does not allow direct downloading of applicant information. For an applicant obtaining 5 Permits to Purchase Handguns, the same information must be entered 5 separate times, once for each separate permit. Firearms Purchaser Identification Cards must be printed in sheets of 8 cards. Information for 8 separate applicants must be entered on the front of each card, then the sheet is printed. It then must be flipped over, and the applicant’s information must be entered onto the back of each of the 8 cards in the sheets. Once your Firearms ID Card and/or Permits to Purchase Handguns are complete, you will be notified by a Police Department Representative to come to Police HQ to pick up your items. Applicants applying for Permits to Purchase Handguns after October 1, 2020 will not receive paper permit forms. They will receive a link to a digital form that will be activated by the Firearms Retailer or other seller when a purchase is being made.

Top Ten Reasons for Delays in Firearms Applications

Applicants not submitting a “Request for Mental Health Records Search” (State Police Form SP-066) to the Hazlet Township Police Department as soon as possible after submitting your online application. This form is only needed for applicants that resided outside of New Jersey at any point during the past 10 years (From 2010 to today). The Search for Mental Health Records for New Jersey is done as part of your online application.  

 

To make this easy for you we have attached a PDF “Request for Mental Health Records Search” form (SP-66) to this email for applicants to complete. Most states will accept this form (EXCEPT NEW YORK- New York Requires their form (OMH-11) completed in addition to the NJ SP-66 form) 

Since many our new residents previously resided in New York State, we have also attached the NY State Office of Mental Health Search form (OMH-11) to this email.  

All completed forms can be dropped off at Police Headquarters or mailed to the following address. NOTE- Original documents only – We cannot accept scanned documents.

First time applicants not setting up an appointment to have their fingerprints scanned at IdentoGO Fingerprint processing sites. We cannot start applicant investigations until we have your fingerprints sent to us from the State Police. 

 Applicants name on the Firearms Application not matching EXACTLY the name that appears on their New Jersey Driver’s License. IdentoGO uses the information from your NJ Driver’s License when they scan your fingerprints. If the name on your Firearms Application does not match your Driver’s License EXACTLY (Including middle initials) your fingerprints will not be matched up with your application

Applicants not listing a full 10-year past address history on their application. All addresses since 2010 must be entered. 

Applicants entering the incorrect Date of Birth on their application

Applicants using relatives as their references and not entering correct emails for references. To process the application the Police Department requires two references from reputable persons who are presently acquainted with the applicant, other than relatives. Not having the feedback will delay the processing. The FARS system sends two reminder emails with the link to submit electronic feedback. (Source NJ State Police FARS Help page). 

Applicants entering incorrect phone numbers or email contact information 

Applicants submitting 2 or 3 applications due to errors in previous applications.  If an applicant makes an error, please email us at Firearms@HazletPD.org before submitting a 2nd application.  Errors that occur with duplicate applications are some of the most difficult to resolve by our investigators. 

Applicants not using Hazlet (07730 zip code) or Hazlet Township (07734 zip code) as the location of your residence.
